Preserving the memory of a loved one for our 'Tree of Life' is a touching and meaningful way of a remembering a loved one and celebrating their life.
Our 'Tree of Life' is a moving and inspirational wall mural and already contains the names of many peoples loved ones many of whom were themselves patients at Heart Care. You can also engrave a leaf for a celebration such as a wedding anniversary or special birthday. You can engrave a leaf and add it to our 'Tree of Life tree at a cost of £200 and the money will go towards helping people in oour local community suffering from heart & lung disease.
How does it work?
- Decide what you would like to put on the leaf
- Purchase a Leaf for our Tree of Life by adding it to your basket
- Print off, complete and return the below form (if you haven't already paid online you will also need to include your cheque) to Rina Guddu, Fund-raiser, Walsall Rehabilitation & Healthy Living Trust, 12 Portland Street, Walsall, WS2 8AB
- If you are a taxpayer please sign the GIFT AID declaration on the form. Your generous gift will be boosted to £250 via our tax reclaim.
