Standing Order

To make paying for your After Care Membership easy and contactless you can set up a Standing Order with your bank, all you need to do is let your bank know our bank details (detailed on the form below) when setting up your standing order.  If you are paying monthly you will need to choose either the 1st or 15th of the month for the money to be sent to us. By paying monthly you save £84!

For any Streetly patients who are not able to print a form please ask the Streetly team for a form.
